The Perry County Health Department is the Vital Registrar for Perry County.

Vital Records:

Death Certificates are available only to: A person, or his duly authorized agent, having genealogical, personal or property right interest in the record. A department of the State or Federal government. Any person upon order of a court.

Death Certificates; Cost $20 for 1st copy, each additional $10.  Per state law, fees for veterans include one free copy and $6 per each additional certificate.
