FCM

Family case management is a voluntary program offered by our facility to promote participants’ lifestyle and health. Our case managers assist by enhancing the participant’s strengths, building needed skills, and connecting with services in the area. The case manager will assess needed health and social services, provide education to clients, and assist with scheduling and/or coordination of referral services. Also, the case manager will follow up to make sure that services have been provided. Such services may include the following:

  • Family Planning
  • Health Education
  • Hearing / Vision
  • Substance Abuse
  • Prenatal / Parenting Classes
  • Child Care
  • Housing
  • Lead Screening
  • Transportation
  • Early Intervention
  • Immunizations
  • Well child exams
  • Health Insurance
  • Domestic Violence
  • Breastfeeding
